Dead space 2 on hardcore even though you can beat it on easy and get every thing there are still necromorphs that sneak up behind you and one shot you and you only have 3 saves in hardcore.
Probably in boss levels in any games. Sometimes there too hard.

The end of Kingdom Hearts. When you get to the end you have to beat Ansem. HE'S SO HARD TO BEAT! Took me hours to beat that on normal. Would take days to beat on hard.
